Entry Fee $60.00

Each Team Guaranteed 3 Games
Game time is 15 minutes or 15 points
Each Player Receives a "Payson Hoops" T-shirt
Individual Trophies for 1^st, 2^nd, 3^rd Places (unless round robin division)
Individual Medals for Consolation Winners

Divisions: K 1-2 3-4 5-6 7-8 9-10 11-12

Girls & Boys Brackets (Team must enter division of player in highest grade level).

There is limited spacing, forms will be accepted on a first come basis.

Entries received after a division is full will be returned

Deadline for entries is March 9, 2012:

Please return as soon possible due to limited spacing.

The team entry fee must accompany the team roster and be postmarked by the entry deadline. No cash refunds after entry deadline.

Current report card, school ID, or yearbook with proof of grade level is required at time of registration.
